[Embedded forms] Add checkbox field

- Please add a checkbox field type in the embedded forms.
- And add a boolean type for custom attribute, so we can map the checkbox with the custom attribute.
- And add ability to map the checkbox field with a tag too (when the checkbox is checked, the tag is added to the contact)
